Duly Accomplished Bond Application Form
Secretary’s Certificate (please see attached format) – Notarized
Co-signer (president, incorporator, director, etc…)

Latest Audited FS w/ BIR Stamp (2010) including the notes to FS
List of Completed and On-going Projects (Name of Client, Project Description, Contract Amount and Project Duration must be indicated)
Government Issued IDs of the signatories and their spouses (Ex. TIN, SSS, etc…)
Bond Requirements – Partnership
Duly Accomplished Bond Application Form
Affidavit executed by one partner attesting his/her consent if the other partner will only be the one to sign on the bond in behalf of the partnership
2 Co-signers (excluding the partners and their spouses)
